    The AHQ is given to every student at the beginning of the school year. It is my way getting updated information from parents, as well as, monitoring for allergies, asthma and other conditions. This form is also an important tool in helping me assess a family need for health insurance. Information is only shared with teachers if you circle YES to question number 7 giving me permission.     In accordance with CIAC regulations 4.15B, all students prior to participating in tryouts and athletic activities, including cheerleading, must have on file, with the school nurse, a current physical examination, which was performed within the past 13 months. The school nurse will inform the athletic director and coach with a list of student compliance.
    Students also need to complete a Concussion and Cardiac form EVERY TIME the student is trying out for a sport.
