The AHQ is given to every student at the beginning of the school year. It is my way getting updated information from parents, as well as, monitoring for allergies, asthma and other conditions. This form is also an important tool in helping me assess a family need for health insurance. Information is only shared with teachers if you circle YES to question number 7 giving me permission. The School Nurse is on duty to assist students who have become ill or injured during school hours. Your family physician should be consulted for illnesses or injuries that occur at home.

All calls to parents/guardians pertaining to illness must be made through the school nurse. Students should not use cell phones or class phones.


A child who shows signs of illness should remain at home. Symptoms of fever, rash, conjunctivitis (pink-eye), vomiting, and persistent sore throat, should be considered reason for keeping a child at home. This allows them to recover, seek medical advice and protects the other students from illness. Any child with a temperature of 100.0 degrees or higher will be sent home. Your family physician should be consulted for illnesses and injuries that occur at home.
